Whistleblowing Report Form

The rules of the form:
 

• Reports made using the form at polimex-mostostal.pl/en/whistleblowing are forwarded to the e-mail address of recipient chosen by the whistleblower, without delay.
• The default addressee of reports is the Ethics Committee of Polimex Mostostal Capital Group, acting in accordance with the Rules of the Ethics Committee, which regulate, among others, scope of the reports under consideration and the procedure for examining reports.
• If the whistleblower considers that due to the nature of the case he does not want to or cannot report to the Ethics Committee, the report may also be submitted to the Head of Internal Audit or the President of the Management Board of "Polimex-Mostostal" S.A.
• In order to allow for a reliable examination and proper reaction to the report, it should contain at least the following elements:
  
  • the nature of the observed irregularity;
  • description of the event;
  • persons involved in the reported irregularity;
  • if possible, indicate documents that could confirm the irregularity.
• The form page does not have any built-in mechanisms to identify the sender. Connections to this page are not logged, and the sender's IP address and MAC number are not identified, saved, processed, shared or transmitted. Such data is also not received by the recipient of the report.
• The form allows to make reports anonymously. However, it should be noted that leaving the contact details by the whistleblower would allow the whistleblower to be notified about the result of his report, and possible contact in a situation where additional information is necessary for the proper processing of the report.
